This is a membership plan not a dental insurance plan and is only valid at Southlake Family Dentistry.
Cannot be used in conjunction with other insurance plans.
Cannot be used in conjunction with other savings or financing plans such as CareCredit®, More Mastercard or 5%-8% decrease in discount to offset merchant fees.
Cannot be used for services covered by other insurance plans including medical, automobile, worker's compensation or other insurance plans not listed.
Patient portion of any treatment is due at time date of service unless prior arrangements have been made.
Membership plans are non-transferrable. Each plan only covers 1 person.
The effective date is the day the annual fee is paid. The renewal date is 365 days from the effective date. Benefits WILL NOT carry over after 365 days of the purchase.
All services, including regular preventive care, must be performed before the endo of the coverage date.
It is the patient's responsibility to make and keep appointments for his/her family members.
